About The Position

North Texas Behavioral Health Authority (NTBHA) is seeking a passionate Care Coordinator to join their team. NTBHA is dedicated to creating a collaborative system of care for individuals with severe mental illness, serious emotional disturbances, and substance use disorders. This role is crucial in assisting clients transitioning from crisis services to outpatient care by developing discharge plans, setting goals, and removing barriers to access. The Care Coordinator will primarily serve Navarro and Ellis Counties, acting as a QMHP-CS at the Corsicana Respite House Program. This position requires strong organizational and clinical skills to ensure continuity of care, potentially involving skills training, case management, and monitoring during community transitions. A key focus will be targeting individual risks and needs to reduce recidivism, hospitalization, and homelessness. Collaboration with internal departments, law enforcement, hospitals, and community agencies is essential. Travel is a significant component of this role, potentially ranging from 20% to 100% daily within NTBHA's catchment area (Dallas, Ellis, Hunt, Kaufman, Navarro & Rockwall Counties). This may involve using a fleet or personal vehicle for field visits, home visits, offsite meetings, and other business needs. Transporting individuals is only permitted with a company vehicle and management approval.

Requirements

  • Bachelor’s degree in psychology, social work, or a related behavioral health field.
  • Must meet the minimum qualifications of a Qualified Mental Health Professional-Community Services (QMHPCS) as outlined in the Texas Administrative Code 353.1415(a)(1).
  • One or more years of direct care experience with persons with serious mental illness and/or substance use disorders (psychiatric crisis service experience preferred).
  • Experience participating as a member of a treatment team that develops and monitors treatment plans.
  • Current knowledge of community resources and support for homeless and indigent populations in NTBHA’s service area.
  • Valid driver’s license.
  • Clear background check as required for healthcare organizations under state and federal contracts.
  • Advanced Microsoft Office Suite skills - Outlook, Word, Excel, PowerPoint, etc., with an ability to become familiar with company-specific programs and software.
  • Must be able to work on and have knowledge of a PC (personal computer).

  • Psychiatric crisis service experience preferred.

Responsibilities

  • Perform coordination work and assist in developing discharge plans, goals, and objectives while eliminating barriers to connect individuals with mental illness and/or co-occurring disorders to outpatient services.
  • Provide screenings, assessments, crisis follow-up, and transitional services that reduce hospitalization, recidivism, or homelessness.
  • Actively participate in staffing’s to determine recovery-oriented treatment.
  • Provide case management, skills training, and monitoring while consumers transition to the community.
  • Collaborate with law enforcement, hospitals, and community providers for service linkages.
  • Maintain accurate records, files, and reports in accordance with policy and standards.
  • Provide trauma-informed, resilience-oriented care in a culturally sensitive manner.
