About The Position

This position focuses on providing case management services that connect individuals and families with the supports they need to improve their well-being. The work involves maintaining close communication with consumers, families, and service providers to ensure progress toward treatment goals. As a County Caseworker 2, you will perform the following duties: Case Coordination: Maintain regular contact with consumers and service providers to review progress and document treatment Support Advocacy: Assist individuals in accessing economic, health, physical, or vocational resources beyond behavioral health needs Direct Engagement: Provide frequent face to face support at least biweekly or as needed based on consumer circumstances Crisis Response: Deliver emergency and crisis intervention services, including participation in rotating 24 hour on call coverage Information Gathering: Collect essential information from consumers and families to identify social, emotional, economic, and health related needs.

Requirements

  • Six months of experience as a County Caseworker 1
  • Successful completion of the County Social Casework Intern program
  • A bachelor's degree with a social welfare major
  • A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences
  • Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require a drug screening.
  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
