About The Position

This opportunity is for County Caseworkers to build rapport and a professional relationship with consumers of all ages in services and their families. Serve as an advocate for the consumer, monitors the delivery of services, links individuals to services, develops social supports, and encourages self-determination. This position provides key support to individuals with developmental delays, mental health needs, or intellectual disabilities. The role focuses on connecting people with essential resources and tracking their progress.

Requirements

• Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.
• You must be able to perform essential job functions.
• You must pass a background investigation.
• Conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

• Service Monitoring: Responsible for monitoring, linking, and evaluating services.
• Link to other services and make referrals as needed
• Provide regular direct individual contact and provide face-to-face case management services while meeting productivity standards as set for the program
• Schedule and facilitate meetings to develop an individualized service plan with involvement of the individual and/or family
• Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy.
• Obtain & update information within required time frames
• Offer crisis intervention services and coordinate assistance as needed.
• Work with Agency staff, providers, and community resources to support individuals
• Link individuals to transportation services or transport individuals to appointments as needed
• Attend meetings, staffings, and trainings as needed.
• Provide coverage in Somerset County.
• Perform other related duties as assigned.
