The Counselor II supports the SolMar Retreat and Hoag Addiction Treatment Center (HATC) treatment team in the assessment, treatment planning, referral recommendations, discharge planning, education, and direct therapeutic intervention for patients and their families throughout the course of treatment. This role supports the goals of the patient’s treatment plan and assists in clinical care and case management. The Counselor II coordinates care with outside providers by referrals when necessary and performs other duties as assigned. For Chemical Dependency and HATC, the role performs focused collaboration with the medical team, admissions, and the utilization review team. For SolMar Retreat, the Counselor II assists with admissions and discharges, assuring all admission and discharge documents are signed. They conduct screenings/intake assessments as needed and within the incidental medical services (IMS) guidelines. The role provides clients with an orientation about the program, rules, and a tour of the unit, including their assigned room. Assistance is provided with searching belongings at the time of admission as well as random room searches in the residential level of care. The Counselor II intervenes as an advocate for clients or patients to resolve emergency problems in crisis situations. They conduct group counseling sessions, manage group sessions as assigned, and lead/facilitate lectures for clients. Ensuring all documentation regarding patient care, group counseling sessions, treatment, and incidents are completed in a timely and clear concise manner is crucial. Participation in case conferences and staff meetings is expected. The role leads and coordinates the community meeting for clients (residential level of care) and assists and encourages clients to participate in self-help groups, such as AA/NA. Communication with HCP/physician, Nurse Practitioner, and Supervisor of Addiction Care regarding physician orders, prescriptions, and refills as needed is required. Observation of self-administration of medications and documentation in client EMR, recording daily living activities and vital information, and reporting patient behaviors to the Supervisor of Addiction Care and Clinical Manager of any changes in behavior are key responsibilities. Receiving and presenting change of shift reports as assigned is also part of the role.
