About The Position
This position involves understanding the impact of social policies on residents' lives and advocating for policies that promote social justice and equity. The role requires engaging in policy analysis and development, with a strong understanding of the policies, procedures, and regulations of skilled nursing facilities. Familiarity with the aging process, chronic illnesses, and disabilities common in long-term care is essential. The position also requires knowledge of available resources and support services for residents and families, and working collaboratively with nurses, physicians, therapists, and other healthcare professionals. Participation in interdisciplinary team meetings and care conferences is expected, along with an understanding of the roles and responsibilities of other team members. The role involves advocating for residents' rights and needs within the facility and with external agencies, empowering residents to make informed decisions and participate in their care, and promoting independence and dignity. Maintaining professional boundaries and confidentiality is crucial.
Requirements
- College graduate with a Bachelors in Social Work or other human services related field.
- 1 year of Long-term care experience
