About The Position

Central Health is seeking a skilled Licensed Clinical Social Worker or Licensed Professional Counselor to join our Behavioral Health Services. The role involves providing compassionate, inclusive, and trauma-informed clinical care with the goal of reducing systemic barriers to mental health and substance use supports, treatment, and services. The Counselor-Behavioral Health Services (Counselor-BHS) will conduct strengths-based diagnostic assessments and provide treatment to support culturally affirming individual, family, and group counseling. Responsibilities also include crisis intervention, patient psychoeducation, advocacy, and clinical consultation. This position reports to the Behavioral Health Manager and services may be delivered in various settings. The ideal candidate will have a commitment to anti-oppressive and trauma-informed care, fostering patient healing through empowerment, transformation, and hope.

Requirements

  • Master's Degree in Social Work or Counseling from an accredited institution.
  • A minimum of 2 years of professional experience performing mental health and substance use assessments, diagnosis, and individual/group treatment.
  • Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC).

Responsibilities

  • Conduct diagnostic assessments of mental health and/or substance use disorders using the DSM, comprehensive clinical assessments, and psychometric screenings to deliver evidence-informed clinical treatments addressing biopsychosocial-spiritual needs, strengths, and goals.
  • Provide trauma-informed, evidence-based, strengths-based, and culturally affirming therapeutic counseling for individuals to enhance overall well-being.
  • Develop care plans and provide services in collaboration with the patient, treatment team, and care providers, consistent with Central Health’s policies, mission, and goals.
  • Lead and facilitate psychoeducational and counseling groups, fostering group cohesion, encouraging participation, and managing group dynamics.
  • Act as a subject matter expert on social work approaches, interventions, and treatment of mental health and substance use disorders, fostering a safe, supportive, and healing environment.
  • Respond to mental health and substance use crises through trauma-informed crisis intervention, de-escalation, and risk assessment, independently or in collaboration with community organizations.
  • Advocate with internal and external stakeholders for patient choices, values, and preferences.
  • Maintain knowledge of community resources and benefits to refer patients and advocate for their needs, priorities, and goals.
  • Support Central Health’s Behavioral Health Program’s continuous quality improvement and utilization management initiatives.
  • Attend and participate in scheduled staff meetings, supervision sessions, training, and other relevant meetings.
  • Complete all direct and indirect patient care documentation and services under professional standards, program procedures, and relevant regulations.
  • Facilitate mental health education for staff on trauma-informed care and behavioral healthcare services.
  • Perform other duties as assigned.
