Make every handoff count. Great hospice care depends on details being handled with care. In this role, you’ll make sure important patient paperwork moves smoothly across multiple agencies—from approved hospice election addendums, to delivery, signature follow-up, documentation, and filing in the medical record. You’ll be the person who notices what’s missing, knows what needs to happen next, and keeps the right people informed. If you enjoy bringing order to busy workflows and seeing an open item all the way through to completion, this could be a great fit. This role will provide: Meaningful work: Your coordination helps support patients, families, care teams, and compliant medical records. Visible ownership: You’ll manage an important process—not just complete isolated tasks. Remote flexibility: Work from home during regular business hours with a private, professional workspace. Collaborative partners: Work with Clinical Directors, operations, compliance, coding, agency leadership, and the ADR and Medical Review team. A clear non-clinical focus: Your strength is organization and follow-through, not clinical decision-making. A role with clear boundaries. This is a non-clinical coordination role. You will work with approved information and keep the process moving. You will not determine clinical relatedness or coverage status, override a Clinical Director’s decision, or create a clinical rationale.
