About The Position

This position is responsible for coordinating and monitoring immunization practices throughout GLBHC, ensuring compliance with vaccine management protocols, and serving as a liaison for all immunization practices. The role involves coordinating immunization clinics, participating in staff onboarding and education, implementing new policies, monitoring quality improvement measures, and maintaining accurate electronic medical record documentation. The position also involves participation in Patient Centered Medical Home recognition activities and other assigned duties.

Requirements

  • Associate’s degree in Nursing preferred, or other health care related degrees or equivalent experience.
  • Basic Life Support (BLS) certification Required.
  • RN, if applicable.
  • Current experience in a health-related field required.
  • Three (3) years of experience in leading teams with project implementation and management or commensurate experience in Patient Centered Medical Home implementation required.
  • Excellent organizational skills and prioritization skills required.
  • Ability to operate office equipment including computer, fax machine, printer/copier.
  • Able to follow through with assignments responsibly, accurately, and in a timely manner.
  • Able to extract data, report data, and evaluate progress/outcomes.
  • Proficiency in using Microsoft Office applications, including Outlook, Word, Excel, and Power Point.
  • Familiarity with using electronic health records and patient database applications.
  • Ability to learn other software applications.
  • Ability to follow through with assignments responsibly, accurately, and efficiently.
  • Demonstrates excellent organizational skills with the ability to manage multiple priorities in a high-volume environment.
  • Demonstrates attention to detail.

Responsibilities

  • Coordinate and monitor immunization practices throughout GLBHC, ensuring compliance with vaccine management including ordering, inventory, storage, handling, temperature monitoring, transfer, administration, and emergency planning.
  • Ensure compliance with VFC (Vaccines for Children) requirements.
  • Serve as a resource for clinical staff and communicate up-to-date immunization recommendations.
  • Investigate all immunization-related medication errors.
  • Coordinate immunization clinics to targeted populations in partnership with other community organizations.
  • Participate in the onboarding of new staff involved in immunization planning, preparation, and administration.
  • Develop and maintain a comprehensive immunization orientation for all new employees.
  • Provide ongoing education related to immunization management and conduct annual immunization competencies.
  • Serve as GLBHC’s liaison for all immunization practices to local health departments, MCIR, MPCA, insurance companies, MDHHS, and immunization manufacturer representatives.
  • Implement and maintain new policies, protocols, and practice management guidelines related to immunizations at GLBHC clinical sites.
  • Monitor quality improvement measures related to immunization practices and coordinate outreach efforts.
  • Work with clinical sites and the Quality Improvement Director to develop strategies to improve immunization rates.
  • Run standard and ad hoc reports, perform data analysis, and develop new reports as needed.
  • Create, edit, and distribute data/reports as directed.
  • Demonstrate knowledge of accurate electronic medical record documentation and assist providers and patient care team members with effective documentation.
  • Assist staff with workflow development in accordance with quality initiatives.
  • Participate in Patient Centered Medical Home recognition activities.
  • Perform other duties as assigned.
