The Coordinator will maintain an active caseload of up to 20-25 individuals referred for START Services. This role involves providing systemic consultation based on assessment and outreach findings, utilizing ecomaps, and offering consultation to systems/teams using best practices. The Coordinator will conduct regular outreach visits to individuals' homes, day programs, or vocational settings for evaluations and assessments, either in-person or via telehealth. They will assist with referrals for consultation and treatment, screen for enrollment in START, and complete intake assessments. The role includes coordinating support meetings and crisis plan development, creating Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, and other required documentation. The Coordinator will participate in recurring consultations, staff meetings, and provide on-call support on a scheduled, rotational basis. Accurate data entry into the START Information Reporting System and other required systems is essential. The position also involves maintaining community partner linkages, developing materials for presentations, and participating in the Center for START Services National Network. Compliance with all necessary trainings, YAI policies, NCSS, and applicable state regulatory agency requirements, including obtaining START Coordinator certification, is mandatory. The role must also comply with Federal, State, Local, and other relevant regulatory agency requirements, including HIPAA.
