About The Position

NYU Langone Health is seeking a Coordinator, Transplant - Lung to support the Transplant Institute. This role involves providing administrative support, including managing patient phone calls, front desk duties, scheduling tests, and facilitating communication with patients and referring providers. The Coordinator will serve as the primary contact for patients, living donors, internal staff, and referring physicians. Additionally, they may assist with the administrative, clinical, and academic needs of faculty members.

Requirements

  • Bachelor's degree with at least 2-3 years of comparable experience or equivalent combination of education and experience.
  • 10 years of work experience in healthcare may be used in lieu of education.
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Responsibilities

  • Demonstrates knowledge of the organization's Service Standards and incorporates them into the performance of duties.
  • Perform administrative duties for the Transplant Institute.
  • Contact patients after initial referral and assures insurance coverage for transplant at this program. Verifies insurance and obtain authorization/referral for consult. Complete all intake duties including gathering information on PCP and referring MD. Ensure patients are on par with providers.
  • Create new patient medical record chart, with all the demographics, history and all forms that are required for the patients consent.
  • Obtain patient records and scan into EPIC with correct labeling to ensure accuracy for providers.
  • Preparing new consults hand out folders for kidney, pancreas, liver, heart or lung patients.
  • Greet patients when they enter the transplant clinic and check in/out.
  • Utilize reporting in order to schedule appointments, testing, consults and obtain records. Upload any files, records, or CDs.
  • Utilizes Phoenix/EPIC checklist and work flow process to coordinate evaluation of donors or recipients.
  • Initiate and obtain prior authorizations as indicated. Update expired authorizations as needed.
  • Facilitate mailings to patients/referring providers and upon request.
  • Mail out new patient packet to the patient with all information for transplant or donation along with forms that may need to be signed by the patient.
  • Assist with faxing and scanning including right fax.
  • Assists coordinators with follow up phone calls to patients to ensure completion of testing for any phase of transplantation (evaluation, waitlist, post-transplant for recipients or donor evaluation and post-donation care).
  • Prepare and distribute the weekly list of the patients that are going to be presented/discussed at the listing meetings.
  • Provides patient education about the transplant and living donation process and what to expect from the transplant center.
  • Assist with tissue typing and lab send outs to American Red Cross and Rogosin Institute.
  • Collaborate with medical assistants (if present) to ensure clinic runs smoothly and efficiently.
  • Answer patient phone calls and address concerns or triage to the correct clinical provider. Understand phone tree and appropriate level of escalation.
  • Reviews and coordinates patient testing and tracks progress in EPIC checklist, synthesize results and reports to transplant coordinators, follows up with patients regarding missing testing.
  • Independently answers questions from patients, donors and referring providers. Escalate clinical symptoms to clinical team.
  • Provides referring physicians and dialysis units information about patients' progress in the transplant system via letters, phone calls, emails, or other forms of communication.
  • In collaboration with transplant coordinator, facilitate work up for patients in any phase of transplantation and make follow up phones calls as needed.
  • Runs EPIC reports of patients requiring follow up and blood work.
  • Assists with locating non-compliant patients who are still in evaluation and with locating patients who are high on the wait list and need to be seen.
  • Additionally, the Coordinator may support the administrative, clinical, and academic needs of our faculty members.
  • Performs any and all other administrative duties assigned.
  • Organize the scheduling of patients independently in coordination with provider’s calendar for those appointments not in a block clinic time or resource.
  • Organize consults for patients and radiology/diagnostic appointments.
  • Perform pre-authorization and peer to peer process.
  • Serve as a resource for Associate Coordinator.
  • Elevated understanding of all transplant phases as to assist clinical transplant coordinators (RNs & NPs).
  • Anticipate needs of patients including lab work, diagnostic testing required per protocols.
  • Serve as escalation of questions and triages unable to be answered by Associate Coordinator.
  • Serve as a preceptor for Associate Coordinator.
