About The Position
The CDRC Patient Disease Coordinator is responsible for coordinating inpatient/outpatient education for patients with Chronic Disease. This role involves developing and applying evidence-based nursing practice standards to achieve optimal patient outcomes. The coordinator oversees evidence-based, individualized, effective care to patients and families in an environment of clinical excellence. They will assist in maintaining a standardized patient education program regarding disease, symptoms, and medications, and aid in the development of a standardized plan of care with measurable goals for patients with chronic disease. The role includes initiating contact with patients in the acute care setting to provide initial/basic education and encourage follow-up post discharge. The coordinator will serve as an educational resource to hospital staff, patients, families, and outpatient primary care physician staff, and will build relationships with community resources to connect patients with services. Responsibilities also include implementing and maintaining education classes and coordinating content experts to deliver education, as well as maintaining a disease-specific patient registry.
Requirements
- Bachelors Nursing required
- Virginia RN licensure required
- Ability to obtain licensure in West Virginia required
- BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required
- Ability to complete Telehealth certification required
- Knowledge and skills to develop adult education programs required
