About The Position

The Coordinator of Day Habilitation Services is responsible for managing, coordinating and supervising the service activities and staff of day habilitation services. The Coordinator of Habilitation Services implements person centered day services and supports to individuals that support their valued outcomes, needs and interests; and ensures quality, continuity and comprehensiveness. The Coordinator of Habilitation Services oversees curriculum development and creates opportunities that promote independence and meaningful opportunities and oversees intake and evaluation for services.

Requirements

  • Bachelor’s Degree in Human Services
  • three years full-time, paid direct service experience with the disabled population
  • two years of supervisory experience
  • Valid New York State Driver’s License in good standing
  • Reliable form of transportation
  • Must have or achieve certification within 3 months of hire in First Aid Certification
  • Must have or achieve certification within 3 months of hire in CPR Certification
  • Must have or achieve certification within 3 months of hire in Ukeru Certification
  • Must have or achieve certification within 3 months of hire in Defensive Driving
  • Comfortable using computers and technology in a professional setting
  • Proficiency in Microsoft Office Suite
  • Ability and willingness to learn and use organizational software systems and electronic documentation platforms

Nice To Haves

  • Master’s Degree in Human Services
  • five years paid direct service paid experience with the disabled population
  • three years of supervisory experience

Responsibilities

  • Provides operational leadership for habilitation services while promoting quality programming and person-centered supports.
  • Supervises and develops staff through training, evaluation, coaching, and performance oversight.
  • Coordinates curriculum, staffing, documentation, schedules, and intake processes effectively.
  • Maintains compliance with OPWDD, Medicaid Waiver, billing, and agency standards.
  • Oversees day habilitation programming, intake coordination, curriculum development, and community integration opportunities.
