About The Position

The Coordinator - Diversion Team is a full-time position requiring 40 hours per week, with a schedule of Monday - Thursday from 7:30 am to 5:30 pm and Friday from 8:00 am to 12:00 pm. This role is based in Tuscaloosa, AL. The position requires proven ability to collaborate with physicians to coordinate care, ask pertinent questions for managing psychiatric and medical care, and coordinate a program that values both aspects for consumer recovery. The role also involves working with other healthcare or community agencies to coordinate the consumer's continuum of care, focusing on both medical and psychiatric needs. Additionally, the coordinator must be able to educate staff and consumers about medications and medical conditions common in intensive psychiatric settings, and assess consumers to differentiate between psychiatric symptoms and potential medical concerns requiring intervention. A comprehensive understanding of medications (medical and psychotropic) and physiological responses to illnesses and treatments is essential. Superior psychotherapeutic skills, excellent collaboration with other program coordinators, and an understanding of training techniques for the SMI population are required. Strong verbal and written communication, organizational, prioritization, and program standard comprehension skills are necessary. The ability to manage, organize, and schedule staff for program success is also key, along with knowledge of cultural diversity and the ability to develop and implement comprehensive treatment plans. Excellent supervisory experience is a must.

Requirements

  • Master's degree in behavioral health (licensure preferred) or RN with 2 years experience in a Mental Health related field required.
  • Must be MAS certified, if RN.
  • One year supervisory experience required, two years preferred.
  • Current Alabama driver's license and good driving record as defined by Indian Rivers' policy.
  • Must be able to read, write legibly, speak, and comprehend English.
  • Proven ability to work as a team with physicians to coordinate care, asking pertinent questions that are required to manage psychiatric and medical care.
  • Ability to coordinate a program that emphasizes both aspects as equally important in the recovery of the consumer.
  • Proven ability to work with other healthcare or community agencies to coordinate all aspects of the consumer's continuum of care, focusing on medical as well as psychiatric needs.
  • Proven ability to educate staff and consumers about medications and medical conditions that often present themselves in intensive psychiatric settings.
  • Proven ability to assess consumers and distinguish whether presenting symptoms are psychiatric or possibly a psychiatric presentation of medical concerns that require intervention.
  • In-depth knowledge of medications: medical – purpose for usage, administration details, possible complications, side effects, interactions; psychotropic – purpose for usage, administration details, possible complications, side effects, interactions.
  • In-depth knowledge of physiological responses to medical and psychiatric illnesses and their treatment.
  • Proven superior psychotherapeutic skills.
  • Excellent ability to work with other program coordinators in a cooperative, supportive manner.

Responsibilities

  • Monitor documentation for the Clinical Program, ensuring admission assessments, treatment plans/reviews, progress notes, discharge summaries, nursing assessments, and medication administration are completed appropriately.
  • Ensure accurate and up-to-date files on all consumers and timely completion of documentation.
  • Supervise staff in identifying medical concerns and developing nursing and therapeutic plans for treatment and monitoring.
  • Provide collaboration and consultation to community agencies as needed.
  • Develop a program for the implementation of well-coordinated follow-up plans for consumers discharged from the program.
  • Complete time sheets.
  • Conduct staff meetings twice per month.
  • Assume lead role in screening consumers coming into the program.
  • Assume lead role in coordinating client care and referring clients to other levels of care within or outside the agency.
  • Conduct intake assessments as needed.
  • Participate in staffing, modeling effective care management and coordinating team participation.
  • Ensure smooth and effective operation and management of programs.
  • Ensure all assigned duties are implemented according to IRBH and DMH policies and procedures.
  • Ensure all state and Medicaid standards for the program are maintained.
  • Ensure all safety, fire prevention, and health measures are followed.
  • Coordinate the transportation of community clients to/from the Clinical Program.
  • Provide supervision of program staff.
  • Supervise nursing staff in coordinating the securing and administering of consumer medications.
  • Schedule staff to ensure adequate coverage of client needs.
  • Work with other coordinators to maintain program movement and recognize progressive movement toward independence.
  • Take an active lead in the development of needed forms and policies, coordinating with other departments.
  • Promote and maintain a safe, therapeutic environment for consumers.
  • Provide accurate documentation of billable time and service reports in a timely manner.
  • Complete monthly administrative reports as required.
  • Develop and maintain MI programming as required by client treatment plans and state/federal guidelines.
  • Devote 40 hours of work per week to the completion of duties outlined in the job description. More than 40 hours may be required.
  • Work with Human Resources on staffing and employee disciplinary issues.
  • Monitor absenteeism or performance problems and implement disciplinary actions in collaboration with HR.
  • Maintain communication and awareness of current events by reading memos, e-mails, bulletin boards, intranet, and communication logs, and responding in a timely manner.
  • Support Organizational Code of Conduct, Standards Compliance, HIPAA and Security Programs by ensuring job tasks are performed legally and ethically, assessing the work area for non-compliance, notifying the supervisor, and maintaining training requirements.
  • Adhere to attendance and punctuality policy, allowing adequate time to arrive, clock-in, and assume responsibilities at the designated time, adhering to meal and break times, and making timely requests for annual leave and reporting the need for sick leave.
  • Comply with all standards related to safety and health by maintaining current TB Test, CPR/First Aid, CPI, and by reporting infections and on-the-job injuries.
  • Identify opportunities for improving the quality of services provided by the department and the organization.
  • Perform on-call duties as needed.
  • Engage in Quality Assurance activities.
  • Perform all other duties and responsibilities as assigned by the Supervisor or Executive Director.
