About The Position

We are hiring two individuals for this role, both fully onsite, with one position based at Midway Elementary School of the Arts in Sanford and the other at Osceola High School in Kissimmee. POSITION SUMMARY Coordinate Community School health and wellness services by identifying student and family needs and facilitating access to medical, behavioral, and support resources. Collaborate with school leadership, families, and community providers to ensure coordinated care, compliance, and effective service delivery.

Requirements

  • Associate’s degree or equivalent combination of experience and education may be considered in lieu of degree.
  • One year of experience in nonprofit, social services, case management, health education, child development, or early intervention preferred.
  • Public speaking experience preferred.
  • Intermediate experience with Microsoft Office required.
  • Customer relationship management database experience, preferred.
  • Customer service skills and experience required.
  • Ability to multi-task and work in a fast-paced environment.
  • Ability to work independently with minimal supervision required.
  • Strong communication skills, both written and orally, required.
  • Ability to utilize critical thinking and problem-solving skills.
  • Ability to utilize organizational skills effectively.
  • Must have access to a private, secure, and stable high-speed internet connection.
  • Must have access to a quiet, confidential workspace.
  • Ability to pass level II background screening.
  • Bilingual is a plus.

Responsibilities

  • Coordinate and track the Community School health care referral network, including medical, dental, vision, and mental health services.
  • Serve as the primary liaison with school leadership, school nurses, health care providers, and internal and external partners.
  • Serve as an initial point of access for students and parents seeking health, wellness, and support services.
  • Participate in multidisciplinary care team meetings to support behavioral interventions, care coordination, and referrals.
  • Track and follow up on all internal and external referrals to ensure service completion and appropriate documentation.
  • Establish and maintain referral partnerships with community agencies and service providers.
  • Complete and maintain required documentation, including assessments, service plans, progress notes, and tracking tools, in compliance with applicable requirements.
  • Participate in crisis intervention and response activities in accordance with established protocols.
  • Oversee eligibility verification, billing, reimbursement, and related administrative processes to ensure compliance and program sustainability.
  • Maintain detailed, accurate and timely records of program data using required databases.
  • Assist with providing detailed reporting on program outputs and outcomes.
  • Attend outreach events to increase knowledge of program services.
  • Meet all grant requirements and expectations.
  • Collect participant success stories to highlight program achievements.
  • Maintain client and staff confidentiality and adhere to HIPAA, FIPA, PHI, PII requirements.
  • May be classified as essential, requiring prompt and effective action during critical situations to ensure the safety and well-being of the community.
  • Contribute to overall organizational success by performing other duties and responsibilities as assigned.
