Coordinator – Community Brain Injury Program, Full-Time Termed
About The Position
We are looking for a Coordinator for the Community Brain Injury Program to join our Therapy Services Team. Therapy Services at the BC Centre for Ability includes the Early Intervention Therapy program, the North Shore School Occupational Therapy Program, the Community Brain Injury Program and SET-BC. The Coordinator of the Community Brain Injury Program (CBIP) is responsible for creating individualized service plans for children and youth with Acquired Brain Injuries across the province. They will coordinate and monitor the recommended therapies that support the child/youth as they transition from in-patient care to their home, school and community. They will identify additional resources for the child and family as appropriate and provide counseling, brain injury education, advocacy and support as needed to the family.
Requirements
- Master's degree in SW
- Ability to coordinate interdisciplinary services for children and youth with acquired brain injuries
- Ability to develop collaborative partnerships with community agencies
- Ability to develop appropriate training material for families, caregivers and professionals
- Practices with a commitment to inclusive practice and family centered care
- Proven collaboration, communication and consultation skills
- Ability to communicate effectively, both verbally and in writing
- Ability to work independently and as part of a team
- Proven organization and time management skills are required
- Ability to adapt to change and embrace new technology
- Competency using a virtual platform, electronic client record system and word processing software
- Must be registered with the BCCSW (regulatory body)
- Valid driver’s license with reliable use of car for work with business insurance
- Physical ability to carry out the duties of the position
Responsibilities
- Review referrals and determine eligibility
- Liaise with community partners regarding discharge planning
- Meet with families and potential clients to complete intake interview and assess rehab needs
- Develop individualized rehab services plan to occur in the client’s home community
- Enroll therapy service providers across the province in the CBIP and engage in service contracts
- Monitor services throughout the service delivery period
- Provide ongoing support and education to the family
- Monitor funding and maintain up to date financial records
- Use Evidence Based Practice and Family Centered Care to guide decision making
- Maintain up to date records that meet agency and regulatory body requirements
- Collaborate with an interdisciplinary team and community partners.
- Participate in staff meetings, case reviews, medical liaison meetings, and in-services
- Undertake additional responsibilities as directed
