About The Position

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our residential clinical care teams in Malvern and/or West Chester, PA. In this role, you will deliver essential direct care, wellness support, and medical facilitation services to clients across designated program locations. You will play a vital role in promoting client health, dignity, independence, and overall physical and psychosocial well-being according to individualized care plans.

Requirements

  • High school diploma or equivalent.
  • Successful completion of a state-approved Nursing Assistant Certification course with an active CNA license/certification in good standing.
  • Minimum of two (2) years of related nursing support experience, preferably in a long-term care or rehabilitation setting.
  • Valid driver’s license with a record of responsible driving.
  • Ability to lift up to 50 lbs.

Responsibilities

  • Provide high-quality personal care, assisting with grooming, bathing, oral hygiene, feeding, toileting, incontinence care, mobility, transferring, and range of motion.
  • Deliver individualized attention that respects each client’s rights, dignity, personal preferences, and freedom of choice.
  • Maintain client skin integrity to prevent pressure ulcers and skin damage through regular repositioning, proper transfer techniques, and application of moisturizers.
  • Promptly respond to client call bells and requests with courtesy and empathy.
  • Support clients during challenging situations (e.g., refusal of activities or care strategies) with patience and professional communication techniques.
  • Perform clinical tasks as assigned, including monitoring vital signs, recording weights, collecting specimens, and applying prescribed creams/ointments.
  • Observe clients for physical or behavioral changes and report observations promptly to the on-call supervisor or clinical team.
  • Assist with care planning by providing feedback to the Residential Care Manager (RCM) and clinical staff regarding client progress and needs.
  • Support and accompany clients at program locations or during off-site medical appointments as required.
  • Complete accurate medical documentation as care occurs, including nursing progress notes, medication log updates, and service logs.
  • Record the date and precise length of each client contact in the system of record.
  • Maintain professional treatment standards, certification requirements, and state regulatory guidelines.
