About The Position

The Coordinated Entry (CE) Coordinator provides leadership and coordination for the Three County CoC’s person-centered, trauma-informed, and equity-focused Coordinated Entry System across Berkshire, Franklin, and Hampshire Counties and the North Quabbin region. The coordinator oversees day-to-day operations, implementation, monitoring, and continuous improvement of Coordinated Entry; supports effective and equitable access to housing resources; facilitates case conferencing; provides training and technical assistance; and works closely with providers, housing partners, people with lived experience, government agencies, and community stakeholders to identify and address system barriers. The position uses data, stakeholder feedback, HMIS, and best practices to strengthen system performance and housing outcomes while ensuring compliance with HUD and CoC requirements. The CE Coordinator reports directly to the Program Director and works collaboratively with the Compliance Manager and other CoC staff, exercising independent judgment, initiative, and problem-solving skills to support an effective, equitable, and responsive Coordinated Entry System.

Requirements

4 years' experience in human services working with low-income individuals or families and vulnerable populations; experience in an office setting including data entry.
Evidence of a good driving record and ability to be covered under Community Action’s non-owned and hired vehicle policy.
Ability to travel throughout the service region and occasionally outside the region

Responsibilities

Manage and coordinate the day-to-day operations of the Three County CoC Coordinated Entry System, including access, assessment, eligibility, prioritization, referral, housing placement, and connection to appropriate resources, ensuring consistent implementation across Berkshire, Franklin, and Hampshire Counties and the North Quabbin region.
Facilitate Coordinated Entry case conferencing meetings across all three counties, including geographic and population-specific meetings, and support timely, equitable, and appropriate referrals and housing placements.
Serve as a primary point of contact for Coordinated Entry questions, referrals, troubleshooting, and system navigation. Proactively identify and resolve routine operational challenges and elevate policy, programmatic, resource, or leadership issues to the Program Director as appropriate.
Maintain By-Names-List, and current knowledge of available all CoC-Funded and non CoC-funded housing resources and programs and coordinate with housing providers and community partners to support appropriate referrals, address housing placement barriers, and improve housing outcomes.
Develop and provide training, technical assistance, and ongoing support to Coordinated Entry providers, assessors, community partners, and system users regarding policies, procedures, assessment, prioritization, referrals, HMIS, and related requirements.
Identify recurring training and technical assistance needs and work with CoC leadership and community partners to develop system-wide solutions, tools, guidance, and resources that strengthen provider capacity and promote consistent Coordinated Entry practices.
Use HMIS and other data systems to monitor Coordinated Entry activity, referrals, housing placements, system flow, data quality, and performance.
Collaborate with CoC staff and providers to support accurate data collection and develop meaningful performance measures.
Maintain accurate records and documentation necessary to demonstrate effective operation of Coordinated Entry and support quality assurance, monitoring, compliance, and continuous improvement activities.
Collaborate with the CoC Compliance Manager to ensure Coordinated Entry operations are consistent with approved CoC policies and procedures, HUD requirements, and applicable federal and state guidance.
Support system-wide activities including the annual Point-in-Time Count, Housing Inventory Count, Coordinated Entry evaluations, community-wide training opportunities, and other CoC initiatives, using information gathered to inform system planning and improvement.
Identify operational barriers, service gaps, disparities, and opportunities for improvement and lead or coordinate efforts to develop, implement, monitor, and evaluate system enhancements using data, stakeholder feedback, and participant experience.
Develop and manage work plans, timelines, objectives, and implementation strategies for Coordinated Entry projects and system improvements, including identifying resource needs, priorities, technical assistance needs, and opportunities for system development.
Participate in the Western Mass Network to End Homelessness through meeting attendance and committee engagement, as directed by the CoC Director.
Promote an equitable, accessible, person-centered, trauma-informed, and culturally responsive Coordinated Entry System. Apply a equity-impact lens to policies, practices, data, prioritization, referrals, and system changes and work to identify and eliminate barriers and disparities.
Staff CoC committees and workgroups related to Coordinated Entry and participate in CoC membership and Board meetings as needed to provide updates, technical expertise, recommendations, and leadership.
Build and maintain strong relationships with funded and non-funded service providers, housing authorities, government agencies, shelters, outreach programs, health and behavioral health providers, youth-serving organizations, victim service providers, veterans' organizations, and other community partners.
Represent the Coordinated Entry System and CoC at community meetings, coalitions, conferences, trainings, and other forums; participate in regional partnerships including the Western Mass Network to End Homelessness; and collaborate with technical assistance providers and external consultants.
Meaningfully engage people with lived experience in Coordinated Entry planning, evaluation, decision-making, and continuous improvement and ensure participant perspectives inform system design and implementation.
Work collaboratively with technical assistance providers.
Required to travel regularly throughout the CoC service area, to CAPV sites, and occasionally out of the service area.
Comply with agency and funders’ paperwork requirements and procedures.
