About The Position

CHOOSE 180 is seeking a dedicated Continuum of Care Program Manager to oversee the day-to-day operations of their Aftercare and Continuum of Care services. This role is crucial in ensuring youth and young adults remain connected to meaningful support following program participation. The Program Manager will contribute to the development and implementation of a continuum of care approach, ensuring sustained support for youth. This includes overseeing the Aftercare and Counseling programs, connecting youth to essential services like behavioral health, mentorship, and community resources, and collaborating with the Chief of Programs to align strategies with organizational objectives. The role also involves conducting program assessments, developing programmatic protocols, and understanding reporting requirements for contracts and funding sources. Additionally, the Program Manager will work collaboratively with other Program Managers to ensure consistency and alignment across all CHOOSE 180 programs.

Requirements

  • 3–5 years of experience working directly with youth or young adults, particularly those impacted by the criminal legal system, community violence, or systemic inequities.
  • 1–2 years of supervision, leadership, or peer support experience.
  • Experience supporting youth through behavioral health, counseling, or trauma-informed programming.
  • Experience working with or within the criminal legal system.
  • Experience in building initial program workplans and operational frameworks that will serve as foundation for a growing department.
  • Experience with Apricot or similar case management database systems
  • Demonstrated success in training and onboarding diverse staff on data entry protocols and system navigation.
  • Strong ability to communicate CHOOSE 180’s mission accurately, effectively, and inspirationally.
  • Entrepreneurial and results-oriented with the ability to anticipate and respond to changing circumstances and priorities.
  • Strong interpersonal and relationship-building skills with the ability to communicate successfully through multiple channels.
  • Strong administrative and organizational skills with attention to detail and process development.
  • Exemplary verbal, written, presentation, and interpersonal communication skills.
  • Must have a valid Washington State driver’s license, insurance, and reliable transportation.
  • Must be able to clear background check requirements necessary for working within partner facilities.

Responsibilities

  • Contribute to the development of a continuum of care approach and lead implementation that ensures youth have access to sustained support beyond initial engagement with CHOOSE 180 programs.
  • Oversee the day-to-day operations of the Aftercare program and Counseling program, ensuring program goals are met and youth are receiving appropriate support.
  • Ensure youth and young adults are connected to behavioral health services, mentorship, community resources, and stabilization supports.
  • Collaborate with the Chief of Programs to implement strategies that are responsive to the needs of program participants and aligned with CHOOSE 180 objectives.
  • Routinely conduct assessments and evaluations of program effectiveness and communicate suggested improvements or program pivots.
  • Support and, in some cases, lead the development of revised programmatic protocols and processes.
  • In collaboration with the Chief of Programs, understand and communicate reporting requirements outlined in contracts and funding sources supporting Aftercare and Counseling services.
  • Work with other Program Managers to ensure alignment, consistency, and collaboration across CHOOSE 180 programs.
  • Provide supervision and leadership to the Counseling Team and Aftercare Coordinators, ensuring youth receive coordinated behavioral health support and case management services.
  • Provide regular supervision, coaching, and professional development support to counseling staff and Aftercare Coordinators.
  • Support the counseling team in navigating complex cases involving trauma, mental health needs, and crisis situations.
  • Provide guidance and oversight to Aftercare Coordinators responsible for connecting youth to resources and maintaining ongoing engagement with participants.
  • Ensure services are delivered using trauma-informed, culturally responsive, and restorative practices.
  • Support staff wellness and address secondary trauma and burnout among team members.
  • Provide case consultation and guidance related to youth support plans and service coordination.
  • Ensure appropriate documentation, ethical standards, and professional practices are maintained across counseling and aftercare services.
  • Participate in relevant stakeholder meetings, including site visits within facilities, schools, and community partner locations.
  • In collaboration with the Chief of Programs, work with external stakeholders to address service gaps and opportunities to enhance programming.
  • Build and maintain strong relationships with community partners, service providers, program partners, participants, and families.
  • Strengthen connections with behavioral health providers and community organizations to expand support networks for youth.
  • As needed, engage in media interviews, panel discussions, and community events to represent the organization’s mission and work.
  • Provide supervision to direct service staff, offering guidance and support in their performance and professional growth.
  • Routinely review and approve timesheets, PTO requests, professional development requests, and mileage reimbursements.
  • Participate in organizational meetings and strategic discussions to contribute to the overall direction and impact of CHOOSE 180.
  • Collaborate with other program managers to support consistency, alignment, and shared learning across programs.
  • Conduct regular data audits and generate monthly impact reports from case management system to visualize program performance.
  • Perform other program-related responsibilities as appropriate.
