About The Position

Continuity of Care Coordinator Are you passionate about making a meaningful impact? Join our team as a Continuity of Care Nav at MHMR, where your expertise will directly contribute to programs that make a lasting impact. As the Continuity of Care Coordinator, you will serve as the liaison between MHMR Tarrant and the Psychiatric Emergency Center (PEC) and coordinates aftercare services for MHMR patients discharging from community hospitals.

Requirements

• Bachelor’s Degree in Human Services Related Field: Applied Behavior Analysis, Child Development, Counseling, Early Childhood Education, Early Childhood Intervention, Education, Psychology, Gerontology, Human Growth and Development, Medicine, Nursing, Physician Assistant, Registered Nurse, Rehabilitation & Rehab Studies, Social Work, Sociology, and Special Education
• Bachelor’s Degree with at least 30 hours of behavioral health coursework which must be documented within the transcript.
• Valid Texas Driver’s License
• Eligible to obtain Qualified Mental Health Professional (QMHP-CS) certification

Responsibilities

• Serve as the liaison between MHMR Tarrant and the Psychiatric Emergency Center (PEC)
• Coordinate aftercare services for MHMR patients discharging from community hospitals.
