Overall Function: Assesses service needs and provides assistance in linking and accessing health and social service systems and interventions as identified through a life domain appraisal. Essential Functions: Acts as the individual/family’s single point of contact for multiple health and social service system linkages, navigation, and referral to address barriers to substance use treatment services and recovery. Links individuals/families to supports and recovery-focused services. Identifies treatment and recovery barriers and ‘walks with the individual’ to navigate through the identified barriers. Monitors the individual/family’s participation in services and provides support to encourage participation. Assists in addressing social determinants of health, which may be a barrier to treatment and recovery. Coordinates and maintains contacts and visits with referral sources and represents the program and agency at various training sessions, community education presentations, and meetings. As necessary, participates in all appropriate staff meetings and maintains beneficial dialogue with all members of the community referral sources, as necessary, regarding assigned clients. Maintains current client case record activity and progress; prepares and submits all required program reports and documentation in an accurate and timely manner. May participate in the staff on-call after-hours rotation process to assist clients, telephonically, with accessing higher emergent levels of treatment care. Adheres to all policies, laws, regulations, and codes of ethics and confidentiality as outlined by federal and state laws, and agency policies and procedures.
