Continuing Care Coordinator
About The Position
The Continuing Care Coordinator is responsible for: The coordination of continuing recovery services for clients returning to the community after completion of the Inpatient Residential Services program and for providing aftercare counseling services to clients on campus. Performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers. Coordinating continuing care recovery referrals by developing positive relationships with community providers and including scheduling of initial appointments. Working closely with the Clinical staff in the Inpatient Residential Services program to ensure the clients transition smoothly from the residential portion of treatment to the continuing care program. Collecting data and program success rates as directed.
Requirements
- Re-integration planning and case management experience required
- Must have a clean driving record
- Must have the ability and willingness to spend up to 50% of their time on local travel using their personal vehicle in the completion of essential duties and responsibilities of the position
- A minimum of 2 years of supervised substance abuse counseling experience required; 3 years or more preferred
Nice To Haves
- LADC, CADC or similar substance abuse treatment certification preferred.
- Bachelor's degree in education-counseling, psychology, rehabilitative counseling, social work, substance abuse treatment, mental health or closely related field preferred.
