About The Position

The Consultative Pharmacist 2 consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy. The Consultative Pharmacist 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action. The Consultative Pharmacist 2 advises safe, accurate and timely usage of medications. Designs, implements, evaluates and makes adjustments to medication care plans. Monitors medication therapy and makes adjustments as needed. Works as the medication expert in focused proactive heath campaigns that target improving health outcomes for patients with specific conditions. Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas. Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed. Follows established guidelines/procedures.

Requirements

Bachelors Degree
Pharmacist license in good standing in OH, AZ, Fl or TX
