The Nurse Practitioner will be responsible for caring for patients and maintaining accurate and current patient records.

Requirements

  • A master’s degree or Ph.D. from an accredited institution of Graduate Medical Education with approved Nurse Practitioner program.
  • Board certified or Board eligible with Board of Registered Nursing (BRN).
  • Active DEA certificate to prescribe in the State of California.
  • Psychiatric Mental Health Nurse Practitioner (PMHNP).
  • Proficient user of Microsoft Office Suite and EHR software.
  • Ability to problem solve and react appropriately to emergency situations.
  • Ability to work independently and effectively collaborate with a team.

Responsibilities

  • Conducts psychiatric assessments and presents them at team and treatment planning meetings.
  • Provides psychiatric services in the office and community-based settings, as clinically indicated.
  • Provides clinical guidance to the treatment team in the comprehensive assessment of psychiatric history.
  • Assesses physical health, making appropriate referral to community physicians.
  • Prescribes psychotropic medications and conducts regular assessments of therapeutic responses and side effects, providing patient education as needed.
  • Provides on-site crisis assessment and is available for on call hours as needed or assigned.
  • Responds appropriately to medical emergencies.
  • Collaborates with inpatient psychiatric providers during client hospitalizations to support continuity of care, medication management, and discharge planning.
  • Complies with formulary as mandated by County Medi-Cal guidelines; participates in Patient Assistance Program to ensure maximum cost-effectiveness.
  • Initiates appropriate and timely interventions on an ongoing basis; provides crisis intervention as needed.
  • Provides documentation per format requirements for individual client’s progress, describing behavior and treatment objectives and outcomes.
  • Assumes an active part in treatment team, providing psychiatric input to the treatment plan on an ongoing basis.
  • Seeks guidance from agency Medical Director or supervising psychiatrist when necessary and implements recommendations and directives.
  • Responds to and cooperates with inquiries and recommendations of Medication Monitoring Committee; makes corrections to documentation and adjustments to procedures as needed.
  • Provides input as required during initial client intake and assessment.
  • Comply with all MHS and program rules, policies and procedures including HIPAA standards.
