About The Position

Adventist Health Columbia Gorge is hiring a Constant Observer to provide direct behavioral support to patients at risk of harm to themselves or others because of mental illness or an inability to care for themselves. The observer works under the direction and supervision of a registered nurse (RN) and leadership, maintaining appropriate supervision by line of sight, as a room sitter, or one-on-one. The role includes recognizing behaviors and statements that require intervention by psychiatric staff.

Requirements

• High school education, GED, or equivalent is preferred.
• Bachelor’s degree or an equivalent combination of education and related experience is preferred.
• Experience in direct patient supervision is preferred.
• Certified Nursing Assistant (CNA) certification is preferred.
• Receive all required vaccinations as a condition of employment and annually thereafter, where applicable; medical and religious exemptions may apply.

Responsibilities

• Monitor patients on involuntary holds initiated by law enforcement or court order and patients identified by a provider as being at risk of suicide.
• Monitor patients who need fall protection, are confused or agitated and at risk of self-harm such as wandering, or may harm themselves by interfering with therapeutic equipment.
• Assist patients with activities of daily living within the scope of practice.
• Monitor changes in patient behavior and report them to the RN or provider.
• Follow safety protocols and infection control policies.
• Perform other job-related duties as assigned.
