You will report into the Manager of Complex Case Management. You will be responsible for overseeing a team of case managers, ensuring high-quality care coordination and support for our members. This role requires a strong clinical background and leadership experience to guide the team effectively.

Requirements

5+ years of experience in case management or clinical care coordination.
Experience leading or supervising a team.
Registered Nurse (RN) license.
Experience working in a health insurance or managed care setting.

Nice To Haves

Experience in a tech or start-up environment.
Certification in Case Management (CCM).

Responsibilities

Supervise and mentor a team of complex case managers, providing guidance and support to ensure effective patient care.
Oversee the development and implementation of individualized care plans for members with complex health needs.
Monitor team performance, ensuring adherence to clinical protocols, quality standards, and regulatory requirements.
Collaborate with healthcare providers, internal teams, and external stakeholders to facilitate seamless care coordination.
Conduct regular case reviews and provide constructive feedback to team members to enhance clinical skills and outcomes.
