Requirements

Master’s degree from an accredited social work program
2 years of experience in an acute or post-acute setting

Responsibilities

Promote patient and family wellness, improved care outcomes, and access to appropriate hospital and community resources.
Manage and support patients with complex and psychosocial needs to ensure a smooth transition from inpatient to ambulatory care settings.
Collaborate with primary care providers, specialties, and the multi-disciplinary Population Health team to provide patient/family advocacy, assessment, discharge planning coordination, and psychosocial interventions for high-risk populations in the outpatient setting.
Manage, support, and develop comprehensive care plans for patients with complex health care needs.
Ensure safety, best practice, and high-quality standards of care are maintained across the continuum.
Collaborate with care management staff to proactively coordinate patients' needs.
