Requirements

  • Master’s degree from an accredited social work program
  • 2 years of experience in an acute or post-acute setting
  • LMSW - Licensed Medical Social Worker - State Licensure or LCSW- License Clinical Social Worker

Responsibilities

  • Promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources.
  • Manage and support patients with complex and psychosocial needs to ensure a smooth transition from inpatient to ambulatory care settings.
  • Collaborate with primary care providers, specialties and our multi-disciplinary Population Health team to provide patient/family advocacy, assessment, discharge planning coordination, and psychosocial interventions for high risk populations in the outpatient setting.
  • Manage, support and develop comprehensive care plans for patients with complex health care needs.
  • Ensure safety, best practice and high quality standards of care are maintained across the continuum.
  • Collaborate with care management staff to proactively coordinate patients' needs.
