Requirements

Master’s degree from an accredited social work program
2 years of experience in an acute or post-acute setting
LMSW - Licensed Medical Social Worker - State Licensure or LCSW- License Clinical Social Worker

Responsibilities

Promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources.
Manage and support patients with complex and psychosocial needs to ensure a smooth transition from inpatient to ambulatory care settings.
Collaborate with primary care providers, specialties and our multi-disciplinary Population Health team to provide patient/family advocacy, assessment, discharge planning coordination, and psychosocial interventions for high risk populations in the outpatient setting.
Manage, support and develop comprehensive care plans for patients with complex health care needs.
Ensure safety, best practice and high quality standards of care are maintained across the continuum.
Collaborate with care management staff to proactively coordinate patients' needs.
