About The Position

Under general supervision, the Chart Completion Tech preps, scans, and validates all types of paper documents into an optical scanning system to obtain chart completion. This role is responsible for scanning and validating all loose reports sent to the department, analyzing medical records for deficiencies in chart documentation, and handling the release of information for continuity of care requests. The position ensures adherence to HIPAA guidelines to minimize the hospital's liability in releasing records or protected health information. Additionally, the role processes requests for government audits and specialized chart reviews (UVMC only), monitors incomplete charts, and follows up with physicians. The Chart Completion Tech must also adhere to guidelines set by JC, Conditions of Participation, and all federal and state regulations. Other duties as assigned and needed are also part of this role.

Requirements

  • High School completion / GED
  • Medical Terminology (completion or enrollment)
  • 1 - 3 years of job-related experience
  • Experience in a Medical Office setting
  • Ability to read and write in order to review and maintain patient medical record charts, and perform basic mathematic calculations as well as ability to operate standard office equipment.
  • Must be able to work independently in a medical office setting.
  • Problem-solving abilities and basic analytical skills necessary.
  • Must possess excellent organizational skills.
  • Ability to type accurately at acceptable levels of speed.
  • Strong verbal and written communication skills.
  • Interpersonal skills required to effectively communicate, understand, interact, and respond to patients, physicians, and other personnel when obtaining and supplying routine factual medical record information via telephone or direct personal contacts on a daily basis.
  • Basic analytical skills necessary in order to determine work priorities, assist in problem resolution and recommend methods of improving workflow.

Responsibilities

  • Responsible for following guidelines of JC and Conditions of Participations and all federal and state.
  • Perform other duties as assigned and needed.
