About The Position

Join Country Health Connect as a Community Support Worker and make a meaningful difference by supporting older people and people living with disability to maintain their independence and quality of life across the Limestone Coast. Working directly with consumers in their homes and communities, you will provide individualised support in accordance with their care plans. This may include personal care, household assistance, meal preparation, transport, shopping and social support. Working autonomously in the community, you will build positive relationships with consumers and families while collaborating with health professionals to provide safe, responsive and person-centred support.

Requirements

  • Hold a current driver’s licence and have access to a reliable, registered and appropriately insured motor vehicle.
  • A current Nationally Coordinated Criminal History Check (NCCHC) for employment involving unsupervised contact with vulnerable groups is required for this position.
  • A satisfactory NDIS Worker Screening Check is required.
  • A current driver’s licence and willingness to drive is required.
  • You must have access to a reliable, registered and insured motor vehicle.
  • You must be flexible and willing to participate in a 7-day roster, working varied hours/shifts across different settings.
  • Immunisation Risk for this position is – Category A.
  • SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.

Responsibilities

  • Provide personal care and practical support, which may include showering, bathing, toileting, personal hygiene, meal preparation, cleaning and laundry.
  • Work effectively with consumers, families, carers, health professionals and other service providers, demonstrating strong communication, reliability and confidentiality.
  • Understand consumer-directed care and reablement principles, supporting consumers to participate in decisions and maintain their independence wherever possible.
  • Recognise and promptly report changes in a consumer’s health or functional status and safely undertake duties in accordance with individual care and service plans.
