Since 2003, Carolina Outreach has been assisting children, families, and adults across North Carolina with their behavioral health needs. We are a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need, utilizing an equitable and inclusive approach. We focus on helping our clients tap into and develop their inherent strengths with the goal of supporting the highest quality of life possible for all we serve. The primary purpose of this position is to provide Community Support Team Services to Mental Health and Substance Abuse consumers. Job duties include but are not limited to: providing assistance and support to consumers in crisis situations; service coordination activities, with or on behalf of a client; psycho-educational and support activities for clients and their families and/or natural supports as appropriate; rehabilitative and independent living skills; and symptom monitoring and management skills. Duties also include informing the client about benefits, community resources, and services; assisting the client in accessing benefits and services; arranging for the client to receive benefits and services; and monitoring the provision of services. This is an off-site job and its purpose is to provide these services in the community to enhance the consumer’s ability to function and achieve their goals.
