About The Position

Carolina Outreach is seeking a Community Support Team (CST) Professional to provide behavioral health services and supports to children, families, and adults across North Carolina. This role focuses on promoting emotional and physical well-being through an equitable and inclusive approach, helping clients develop their inherent strengths to achieve the highest quality of life. The primary purpose of this position is to provide Community Support Team Services to Mental Health and Substance Abuse consumers in the community to enhance their ability to function and achieve their goals. Duties include assisting consumers in crisis, service coordination, psycho-educational and support activities, rehabilitative and independent living skills training, and symptom monitoring and management. The role also involves informing clients about benefits and community resources, assisting them in accessing these services, and monitoring their provision.

Requirements

  • Bachelor's degree from an accredited university in a Human Services field with 2 (two) years experience working with the population to be served.
  • Bachelor’s Degree in a non-Human Service field with 4 years full-time, post-bachelor’s degree experience with the population served.
  • Master’s degree in human services with 1 year experience working with population served.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Must complete the state mandated Community Support Team training within specified timeframes.
  • Must be authorized to work in the United States.

Responsibilities

  • Serve as a member of the client’s Community Support Team in the implementation of the consumer’s Person-Centered Plan.
  • Assist in planning, scheduling and implementation of services for the clients.
  • Work to implement service delivery with other staff and/or across agencies.
  • Develop therapeutic interventions to work with consumers.
  • Provide any written documentation as required, including treatment notes, goals, and plans for the medical record (e.g., Service notes, input into person-centered plan, etc.).
  • Provide individual, family, and/or group therapy sessions.
  • Maintain accurate and timely clinical documentation.
  • Collaborate with care teams as needed.
  • Maintain compliance with all clinical and regulatory standards.
  • Participate in supervision (if applicable based on licensure level).
