About The Position

Carolina Outreach is seeking a full-time Community Support Team (CST) Professional Team Lead to join their team. This role provides essential expertise and consultation to promote a culture that respects and integrates each client’s preferences, values, and unique perspectives into their treatment plans. The goal is to support clients in achieving symptom stability, improving their quality of life, and fostering independence within their communities. The position involves conducting comprehensive initial assessments and reassessments, providing individual and family therapy using evidence-based practices, developing and implementing person-centered plans, monitoring and managing symptoms, and facilitating referrals to higher levels of care. Case management duties include assisting clients in accessing and coordinating benefits, resources, and services, acting as a liaison between clients, families, and community resources, and educating clients and families on available community resources. Services are delivered in off-site, community-based settings, and prompt crisis response is expected. Accurate and timely documentation, including service notes, treatment plans, and progress reports, is required, along with monitoring and evaluating the quality of services provided. This role is compensated on a variable rate model, designed to reward productivity while providing consistent earning opportunity through established referral volume. Compensation ranges from $57,807 to $77,904, based on licensure level, education, and experience. This is a variable rate non-exempt position with unlimited earning potential.

Requirements

  • Master’s Degree in Counseling, Social Work, Psychology, or a related field.
  • Provisional or Full Mental Health License in NC (LCSW/A, LCMHC/A).
  • Minimum of one year working with the population served.
  • Strong organizational, communication, and problem-solving skills.
  • Must be authorized to work in the United States.

Responsibilities

  • Conduct comprehensive initial assessments and reassessments to evaluate client needs and strengths.
  • Provide individual and family therapy using evidence-based practices to support the mental health and well-being of clients.
  • Develop and implement person-centered plans to guide service delivery.
  • Monitor and manage symptoms through psycho-education and therapeutic interventions.
  • Facilitate client referrals to higher levels of care, such as ACT services, when appropriate.
  • Assist clients in accessing and coordinating benefits, resources, and services in the community.
  • Act as a liaison between clients, families, and community resources to ensure service continuity and effectiveness.
  • Educate clients and families on available community resources, benefits, and service options.
  • Provide ongoing support for rehabilitative and independent living skills development.
  • Deliver services in off-site, community-based settings to foster client engagement and success.
  • Respond to client crises promptly and effectively, providing support and guidance as needed.
  • Maintain accurate, timely, and compliant documentation, including service notes, treatment plans, and progress reports.
  • Monitor and evaluate the quality of services provided to ensure compliance with agency standards and client needs.
  • Provide individual, family, and/or group therapy sessions.
  • Maintain accurate and timely clinical documentation.
  • Collaborate with care teams as needed.
  • Maintain compliance with all clinical and regulatory standards.
  • Participate in supervision (if applicable based on licensure level).
