About The Position

Carolina Outreach is seeking a full-time Peer Support Professional to join their Community Support Team (CST). This role involves providing clinical services to clients receiving CST team services, including tenancy support interventions using the Permanent Supportive Housing model, service coordination, symptom assessment and management, individual supportive counseling, providing activities-of-daily-living, social, interpersonal relationship, and leisure-time-activity services, support services or direct assistance to ensure clients obtain basic necessities, and education, support, and consultation to clients' families and other major supports. The position requires adherence to treatment models such as Permanent Supportive Housing, CBT strategies, MI, and Harm Reduction. The role is compensated on a variable rate model based on productivity and established referral volume, with potential for unlimited earnings. Requirements

  • Must have completed certification program for Peer Support Specialists.
  • Must have at least two years of sustained recovery from any identified mental health or substance use issues.
  • Must become NVCI certified.
  • Must have received training in the population served and client rights/confidentiality.
  • A valid driver’s license and current automobile insurance is also required.
  • Must be free from all illicit substance use.
  • Must be authorized to work in the United States.

Nice To Haves

  • Demonstrate adherence to the treatment models within CST such as Permanent Supportive Housing, CBT strategies, MI, and Harm Reduction.
  • At the discretion of team lead, supervisor, and/or management, this position may require additional duties not specifically listed above.

Responsibilities

  • Assist in identifying, planning, scheduling, and implementing services for the client.
  • Assist in the development of transition plans when clients move in and out of services.
  • Act as an advocate in the community.
  • Provide any written documentation as required, including treatment notes, goals, and plans for the medical record.
  • Be available to respond as needed to clients and co-workers 24/7/365.
  • Maintain accurate and timely clinical documentation.
  • Collaborate with care teams as needed.
  • Maintain compliance with all clinical and regulatory standards.
  • Participate in supervision (if applicable based on licensure level).
