About The Position

Valley-Wide Health Systems is looking for a Community Support Specialist who is passionate about helping others and making a meaningful impact in the community. This role provides support, care coordination, and life skills assistance to help patients navigate challenges and improve overall well-being.

Requirements

  • High school diploma or equivalent
  • Personal lived experience related to recovery, mental health, trauma, or supporting a minor with addiction or mental health conditions
  • Colorado Peer and Family Specialist Certification (CPFS), National Certified Peer Recovery Support Specialist Certification (NCPRSS), or ability to obtain certification within one year of hire
  • Valid Colorado driver’s license and willingness to travel
  • Ability to pass a Behavioral Health background investigation

Nice To Haves

  • Bachelor’s degree in social work, counseling, psychology, or a related field from an accredited institution.
  • Applicants with a non-related Bachelor’s degree may be considered with relevant healthcare, human services, or customer service experience
  • Strong communication, organization, and problem-solving skills
  • Ability to work independently while maintaining a collaborative, team-based mindset
  • Familiarity with community resources and agencies
  • Strong attention to detail and ability to manage multiple priorities

Responsibilities

  • Provide supportive, patient-centered services to individuals receiving behavioral health care
  • Help patients navigate resources, appointments, treatment plans, and community services
  • Support patients in building daily living, coping, and wellness skills that encourage independence and stability
  • Facilitate and participate in individual and group-based support services and skills training
  • Coordinate care with providers, clinical teams, community organizations, and outside agencies
  • Maintain ongoing communication and follow-up with assigned patients to support continuity of care
  • Complete timely and accurate documentation, treatment plan updates, and required reporting
  • Participate in team meetings, trainings, and supervision opportunities
  • Build positive, professional relationships with patients, families, coworkers, and community partner
  • Promote a respectful, inclusive, and culturally responsive care environment
