About The Position

Community Support Specialists (CSS) provide mental health and substance use treatment support in a person’s community. This role is with HOP- Westside Safe Haven (WSH), a 13-unit low-barrier housing program offering on-site clinical supportive services for individuals experiencing homelessness with severe mental illness. These individuals often struggle to access services and/or housing due to their mental health symptoms, leading to chronic unsheltered homelessness and increased risk. Westside Safe Haven utilizes a housing first, trauma-informed, and client-centered approach to meet immediate needs and assist individuals in achieving their goals. As a Community Support Specialist, you will be involved in care planning, helping clients grow and recover. You will remove barriers to mental healthcare through case management, connecting clients with resources, and navigating complex systems. You will also help clients develop daily living skills by providing transportation for appointments, benefits offices, or grocery shopping. The role requires adaptability to client schedules, needs, and treatment plans. Requirements

  • Effective communication and relationship-building skills
  • Good writing skills
  • Strong time management and organization skills
  • Must be at least 21 years old
  • A valid driver’s license, current car insurance, and daily access to your own car
  • Willingness to provide transportation to clients
  • Must obtain and maintain liability car insurance of: $100,000 per person bodily injury, $300,000 per accident, $100,000 property damage
  • High school diploma OR GED certificate AND 5 years of supervised clinical experience
  • Bachelor’s degree in Psychology, Social Work, or related field
  • Bachelor’s degree in an unrelated field AND 2 years of supervised clinical experience
  • Master’s degree in Psychology, Social Work, or related field

Responsibilities

  • Develops and reviews members’ care plans by identifying goals, objectives, and specific mental health services needed.
  • Teaches members independent and community living skills, such as housekeeping, meal preparation, coping skills, etc.
  • Provides support to members with collateral social service agencies and the community.
  • Documents services provided in an Electronic Health Record in a timely manner.
