This position requires working with individuals with mental illnesses (who may also have a co-occurring substance use disorder) in achieving and maintaining the highest degree of independent functioning. A comprehensive array of rehabilitation services are provided, much of which are in the community. Assessing client needs, planning and linkage to appropriate services, advocacy as well as providing outreach and engagement are essential job functions. Coordinate and provide services within an integrated and recovery oriented system of care.
