The Community Support Specialist will inspire hope, employ an individualized and multidisciplinary approach to care, and assist patients in identifying and seeking treatment for underlying health conditions that perpetuate addictive behavior. The role focuses on helping patients achieve independent functioning and rehabilitation using a community-based model and evidence-based interventions. Patient care emphasizes personal strength, skills, and harm reduction, using motivational approaches to promote self-advocacy and recovery participation. The specialist will facilitate recovery, identify and teach patients to use their strengths and resources, and provide education on substance use disorders and prescribed medications. This role involves providing individual assistance to patients adhering to their medication regimen, working independently in patient homes and community settings, and encouraging recovery efforts, independence, self-care, and responsibility. The specialist will participate in developing individual treatment and rehabilitation plans, provide individual support for emergency services (financial, medical, housing), and follow up on referrals. They will utilize advanced knowledge of psychological concepts, evidence-based interventions, and coordination of mental health and community services. Responsibilities include ongoing patient assessments, clear documentation of services provided, and identifying risk factors for relapse with a plan to support recovery.
