CLP/TDSO Community Support Professional – Adults with Developmental Disabilities (San Diego)
About The Position
Home of Guiding Hands is seeking CLP/TDSO Community Support Professionals to work one-on-one with adults with intellectual and developmental disabilities in San Diego County. This role offers flexible, part-time work focused on helping individuals build life skills, achieve personal goals, participate in their community, and increase their independence. The position involves direct work with individuals in their homes and in the community, engaging in activities such as exploring local attractions, attending college courses or exercise classes, volunteering, participating in art programs, practicing daily living skills, and fostering social connections. The ultimate goal is to enhance each person's independence, confidence, and community involvement.
Requirements
- High school diploma or GED
- Must be at least 18 years old
- Ability to pass a DOJ background check
- Valid California driver's license with a safe driving record and at least two years of driving experience
- Safe and reliable vehicle for transporting clients
- Current auto insurance
- CPR/First Aid certification or willingness to obtain it
- Comfortable using email, text messaging, websites, and apps
- Able to work independently and manage your schedule
- Reliable with good judgment and communication skills
Nice To Haves
- Experience supporting individuals with special needs is preferred, but not required
- Personal, volunteer, or paid experience is welcome
- Willingness to obtain CPR/First Aid certification (training is available for new hires)
Responsibilities
- Plan and conduct visits with assigned clients
- Help individuals access and participate in their community
- Support clients in working toward their Individual Service Plan (ISP) goals
- Work with clients to develop meaningful personal goals
- Advocate for the individuals you support
- Attend and participate in ISP meetings
- Communicate with family members and other members of the individual's support team
- Complete required documentation and reports
- Encourage independence, choice, and community involvement
- Develop goals with clients (TDSO specific)
- Prepare ISP reports (TDSO specific)
- Coordinate with the individual's support team (TDSO specific)
- Maintain required documentation (TDSO specific)
