About The Position

Home of Guiding Hands is seeking CLP/TDSO Community Support Professionals to provide one-on-one support to adults with intellectual and developmental disabilities. The Community Living Program (CLP) and Tailored Day Services Option (TDSO) offer individualized support, helping individuals build life skills, achieve personal goals, engage in their community, and increase independence. This role involves direct work with individuals in their homes and community settings, focusing on activities meaningful to them to foster independence and confidence.

Requirements

High school diploma or GED
Must be at least 18 years old
Ability to pass a DOJ background check
Valid California driver's license with a safe driving record and at least two years of driving experience
Safe and reliable vehicle for transporting clients
Current auto insurance
CPR/First Aid certification or willingness to obtain it
Comfortable using email, text messaging, websites, and apps
Able to work independently and manage your schedule
Reliable with good judgment and communication skills

Nice To Haves

Experience supporting individuals with special needs is preferred, but not required
Personal, volunteer, or paid experience is welcome
Willingness to obtain CPR/First Aid certification (training is available)

Responsibilities

Plan and conduct visits with assigned clients
Help individuals access and participate in their community
Support clients in working toward their Individual Service Plan (ISP) goals
Work with clients to develop meaningful personal goals
Advocate for the individuals you support
Attend and participate in ISP meetings
Communicate with family members and other members of the individual's support team
Complete required documentation and reports
Encourage independence, choice, and community involvement
Develop goals with clients (TDSO specific)
Prepare ISP reports (TDSO specific)
Coordinate with the individual's support team (TDSO specific)
Maintain required documentation (TDSO specific)
