About The Position

This position is responsible for working directly with the youth, and in collaboration with their guardians (when appropriate), case workers, and community partners, the Loft Care Coordinator, and the Walla Walla Youth Services team, to facilitate care coordination, referral services, and support youth in developing goals that promote wellness, stability, and community connection.

Requirements

  • A bachelor’s degree in social services or a closely related field from an accredited school.
  • Two years of experience working with children or youth.
  • Knowledge and/or experience in providing case management services.
  • Experience in direct client services.
  • A valid Driver’s License and the ability to drive for work use.
  • Ability to adhere to an appropriate self-care plan.
  • Working knowledge of: Word Processing software, Spreadsheet software, Internet software.
  • Reliable employee transportation is a must as this position will require off-site meetings.

Responsibilities

  • Assist in the development of procedures and practices for Youth Services programming in alignment with state and national best practices.
  • Coordinate with Youth Services leadership to support community outreach, partnership development, and program collaboration.

  • Convene Community Support Teams for the purpose of identifying support for youth (under age 18) focused on resolving family conflict and obtaining or maintaining long-term and stable housing as described in House Bill 1406 (sec 5).
  • Work in collaboration with Youth Services leadership, the Loft Care Coordinator, and other Youth Services team members to determine youth’s current needs and barriers, create a community support team, and schedule and facilitate multidisciplinary team meetings.
  • Create an agenda for community support team meetings with youth and send it out to those invited beforehand.
  • Support a safe and secure environment for youth, staff, and visitors in Youth Services settings.
  • Coordinate with community partners, other Youth Services staff and youth’s support system to develop, implement, and oversee services, supports, and activities that promote youth stability, skill building, and community connection.
  • Interface with other relevant resources and our local system of care to secure long-term resources and other needed services for the youth we serve.
  • Develop and maintain strong community partnerships and up-to-date knowledge of resources for youth experiencing homelessness or other barriers.
  • Act as an advocate for youth with community agencies and collaborate with the Youth Services team to support engagement and connection to services.
  • Create and maintain a case management file for each youth, documenting case management services received and progress toward goals in accordance with Catholic Charities policies and procedures.
  • Meet with youth to review progress toward goals, identify current action steps needed to achieve goals, and coordinate with the Loft Care Coordinator and Youth Services team as needed to support continuity of care.
  • Ensure client confidentiality according to Catholic Charities policies and procedures.
  • Enter data into established entry system(s), exit and follow up, as required; provide monthly reports documenting activities for funding sources and attend required meetings as applicable.
  • Ensure client data in HMIS is current, accurate, and entered in real time.
  • Maintain appropriate professional boundaries with youth, their families, community partners, and co-workers.
  • Ability to adhere to an appropriate self-care plan.
  • Use appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
  • Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.
  • As a mandated reporter, follow all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.
  • Adhere to the tenets of Catholic Social Teaching and Catholic Doctrine.
