About The Position

This position is responsible for community case support of participants active in an accountability court program. This position will work directly with individuals diagnosed with substance use and/or other mental health disorders involved in the criminal justice system. This position will conduct home visits, assist participants in accessing transportation, support participants in seeking vocational opportunities, help ensure attendance at treatment and court sessions, and conduct medication counts to ensure compliance.

Requirements

One year full-time (or equivalent part-time) work experience in social services delivery (case management, outreach, prevention/education, etc.) or one year of full-time work experience performing direct care activities, basic nursing procedures, and training for clients at a level of a health services technician.
An Associate’s degree in social work, psychology, nursing, and/or education is required.
Incumbent must be able to work in the fast-paced environment of the courts under limited supervision.
Individual must have knowledge of Microsoft Suite.
Valid Georgia driver’s license.
General knowledge of County organizational structure and complex Treatment Services operations;
Demonstrated proficiency in oral and written communications;
Extensive experience and skill in developing effective working relationships with other departments and the general public;
Demonstrated skill in developing and maintaining positive customer relations;
Knowledge of mental health and/or substance use disorders and effective treatment;
Extensive skill in operating a personal computer, various software applications, Internet, multimedia programs, and calculator.

Responsibilities

Provides limited para—professional social services to participants
Provides administrative support to the treatment staff members
Updates and maintains accurate information regarding participant’s healthcare and social background
May, under professional supervision, provide psychoeducational services to participants
Prepares, updates, and maintains clear and concise notes in participant’s charges, as required by departmental policy
Provides appropriate input in developing and maintaining a network of community contacts
Helps participants perform community integration activities
Attends and participates in treatment team meetings and court sessions
Assists in planning and implementing effective activities for participants
Assists in monitoring participant’s progress towards treatment goals
Provides referrals to facilitate continuity of care
Teaches daily living skills to clients and monitors/documents progress
Focuses on skill development and assists with linkage to resources in the community
Transports participants, as needed. Maintains accurate mileage log according to departmental policy. A county vehicle will be provided for such use. Will notify Director of maintenance issues with county vehicle and will assist in scheduling regular appointments for regular maintenance of upkeep of vehicle.
Makes visits to participant’s homes to ensure ongoing safe and appropriate living conditions as determined by program policy
Conducts medication counts with all participants and reports and compliance issues to team
Maintains confidentiality of all participants and office information as required by Federal law
Prepares and submits required reports and other documentation, as requested
Provides appropriate emotional support and encouragement to participants and assist in the resolution of minor problems
Assists participants in locating and utilizing community resources, included legal, medical, financial assistance, and other referral services as needed
Works under the supervision of the Program Coordinator
Assists with drug testing collection as needed
Regular and predictable attendance is required
Performs other duties as required
