Requirements

Bachelor's degree in Human Services or related field from an accredited institution.
Minimum of 1 year of experience in human services.
Experience working with families in crisis situations.
Obtain and maintain SOAR (SSI/SSDI Outreach, Access and Recovery) credentials.
Ability to work effectively with diverse cultures, family systems, and social backgrounds.
Proficiency in Microsoft Office applications.
Strong verbal and written communication skills.
Passage of background clearance, valid driver's license, reliable transportation, and proof of liability insurance.

Responsibilities

Provide crisis intervention, assessment, and case management support services.
Assess individuals and families for housing-related services and enter qualifying clients into the Coordinated Entry System.
Facilitate educational series and workshops for community members and clients.
Implement a range of support services based on client needs.
Provide case management and referrals for individuals with mental, emotional, or substance abuse issues.
Collaborate with case managers and counselors to plan and coordinate services.
Ensure compliance with McKinney-Vento laws regarding education of homeless students.
Review client records and documentation for compliance with various standards.
Distribute and evaluate client satisfaction surveys.
Assist with HMIS data quality monitoring.
Manage caseload of households, providing ongoing support services including case management, goal planning, resource coordination, and advocacy to promote housing stability and self-sufficiency.
Other duties as assigned within the guidelines of this position.
