Requirements

  • Bachelor's degree in Human Services or related field from an accredited institution.
  • Minimum of 1 year of experience in human services.
  • Experience working with families in crisis situations.
  • Obtain and maintain SOAR (SSI/SSDI Outreach, Access and Recovery) credentials.
  • Ability to work effectively with diverse cultures, family systems, and social backgrounds.
  • Proficiency in Microsoft Office applications.
  • Strong verbal and written communication skills.
  • Passage of background clearance, valid driver's license, reliable transportation, and proof of liability insurance.

Responsibilities

  • Provide crisis intervention, assessment, and case management support services.
  • Assess individuals and families for housing-related services and enter qualifying clients into the Coordinated Entry System.
  • Facilitate educational series and workshops for community members and clients.
  • Implement a range of support services based on client needs.
  • Provide case management and referrals for individuals with mental, emotional, or substance abuse issues.
  • Collaborate with case managers and counselors to plan and coordinate services.
  • Ensure compliance with McKinney-Vento laws regarding education of homeless students.
  • Review client records and documentation for compliance with various standards.
  • Distribute and evaluate client satisfaction surveys.
  • Assist with HMIS data quality monitoring.
  • Manage caseload of households, providing ongoing support services including case management, goal planning, resource coordination, and advocacy to promote housing stability and self-sufficiency.
  • Other duties as assigned within the guidelines of this position.
