About The Position

Responsible for coordination and oversight of Federal, State, and private grant funded programs including but not limited to Aging and Disability Resource Center, Senior Health & Wellness, Area Plan Grants, Care Transitions, and Benefit Enrollment Center. This role also acts as CLC System Administrator.

Requirements

  • Bachelor’s Degree in Social Work, Human Services or related field and two years of experience providing direct human services.
  • Possession of a valid driver’s license and minimum state-required vehicle insurance and have use of reliable transportation
  • Ability to plan, organize, prioritize, and coordinate work assignments and/or projects.
  • Ability to develop program policies and procedures for new service approaches and/or new areas of responsibility.
  • Ability to make presentations, lead, facilitate or participate in meetings, work groups and support groups to achieve desired results.
  • Ability to travel to and from meetings and other community events, which might not be ADA accessible.
  • Ability to work effectively as a team member with a wide range of diverse staff, contractors, and community members to establish and maintain effective working relationships.
  • Ability to communicate verbally in the English language in face-to-face in one-on-one settings, group settings, by personal computer or using a telephone.
  • Ability to produce written documents with clearly organized thoughts using proper English sentence construction, punctuation, and grammar.
  • Computer and software skills to include but not limited to ; ability to operate general office equipment; work at a desk using phone and computer for up to eight hours a day.
  • Ability to perform basic math functions such as calculating percentages.

Responsibilities

  • Coordinates community-based programs for older adults, adults with disabilities and family caregivers.
  • Participates with other program staff to contribute to the development of the four-year area plan and procurement of services.
  • Acts as the subject matter expert for assigned programs.
  • Provides program policies and procedures training and technical assistance to AAADSW staff and contractors.
  • Implements and amends program policies, procedures and develops required reports.
  • Reviews Home and Community Living Administration Management Bulletins and develops draft communication to inform stakeholders.
  • Reviews and approves Exceptions to Policy requests.
  • Monitors CLC service enrollments, analyzes data, and provides feedback for variances or discrepancies.
  • Reviews OAA and SFY budgets and other fund sources to allocate funding for program expenditures.
  • Reviews and approves program expenditures and contractor invoices.
  • Monitors contractor performance to ensure compliance with program standards, minimum service levels are met, contractors are spending to allocation and programs run efficiently and effectively.
  • Coordinates with Contracts Department to develop, review and evaluate Requests for Qualifications and Proposals.
  • Completes Contractor Risk Assessments.
  • Writes Comprehensive, Focused and Quarterly Monitoring reports.
  • Conducts quality assurance reviews of programs.
  • Collaborates with Contracts Department to develop content for contract amendments.
  • Responds to and resolves service delivery issues
  • Collaborates with community partners to enhance and expand programs.
