About The Position

Open Sky’s Community Supports Program is a Mass Health funded navigator service that provides case management and care coordination to individuals in the community. Individuals served in the program must have a mental health disorder or co-occurring disorder and be considered at high risk for admission to a 24-hour treatment setting. The Navigator Supervisor provides direct service, clinical, administrative support, and operational oversight within Community Services programs. They support quality, consistency, and staff development by supervising Navigators and/or managing core processes such as intake coordination, outreach workflows, and onboarding while serving as a bridge between frontline staff and program leadership, ensuring timely communication, adherence to program standards, and effective service delivery.

Requirements

  • High School Diploma, GED or equivalent, required.
  • Valid Driver’s License with acceptable driving history and reliable, insured transportation, required.
  • Minimum of three years experience working with, or lived experience with homelessness, justice involvement, adults with serious and persistent mental illness, substance use disorders, and underserved populations, required.
  • Demonstrated experience providing leadership, oversight, or informal supervision, required.

Responsibilities

  • Provide mentoring, training, coaching, and support to a small number of Community Services Navigators, as assigned.
  • Complete all Assessments and Support Plans according to prescribed timelines and specifications.
  • Work collaboratively with medical and behavioral health providers to obtain any active comprehensive care plans that the member may have.
  • Develop a Support Plan with members which guides services in a way that is consistent and supportive of other existing comprehensive care plans.
