To educate persons diagnosed with a dementia and caregivers, facilitate access to available public and private resources and to provide support services including counseling and care planning. This role involves understanding mental health issues common among persons with dementia and their caregivers, utilizing effective non-pharmacological interventions for managing dementia-related behaviors and common mental health disorders, and implementing strategies to reduce caregiver burden and improve their physical and mental health. The coordinator will also identify and leverage available educational and informational resources. A key aspect of this role is interacting with patients and families in a way that is sensitive to their individual needs, health literacy levels, and home environments, while also assessing any financial burdens that might affect their access to or affordability of services. The position requires the ability to use a care consultation system, transfer assessment information to designated individuals, and efficiently administer and interpret standardized patient and caregiver needs assessment tools. The results of these assessments will be integrated into a comprehensive plan of care for both the patient and caregiver. The coordinator will assess caregiver burden, mental health, and their understanding of the disease and their capacity to manage the caregiving role. This includes providing for the social needs of the patient/family, completing psychosocial assessments, and collaborating with team members in care planning. The role also involves finding alternatives to home care when necessary, documenting according to policy, and treating patients and families with compassion. The coordinator will base care decisions on the plan of care, monitor and document patient responses, establish rapport with clients, adapt communication styles, listen to and respond to patient needs, recognize age-specific responses to treatment, provide effective interventions, and respect patient confidentiality and privacy. Additionally, the position supports the social, spiritual, and cultural needs of patients and families, and manages difficult or emotional patient situations. Additional duties include adhering to hospital and departmental attendance and punctuality guidelines, performing all job responsibilities in alignment with the organization's core values, mission, and vision, and performing other duties as required. Maintaining current knowledge in areas of responsibility, including specialty certification requirements through self-education and ongoing educational programs, is essential. The coordinator will attend staff meetings, complete mandatory in-services and requirements, and competency evaluations on time. Competency must be demonstrated at all levels in providing care to all patients based on age, sex, weight, and demonstrated needs. For non-clinical areas, training and demonstration of age-specific customer service skills are required. Protective clothing and equipment will be worn as appropriate.
