About The Position

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area. With a staff of 500 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail. We are currently seeking a Community Service Worker to join our Community Support Services team.

Requirements

  • Degree or diploma in Social Services or related fields with at least two years of relevant experience
  • Knowledge of available community resources and supports in the GTA
  • Excellent customer services, communication and problem solving skill
  • Ability to work independently a well collaboratively as part of a team
  • Fluency in written and spoken English, as well a second language commonly used by clientele, such as Cantonese or Mandarin is preferable
  • Valid vulnerable sector police record check completed within one year

Nice To Haves

  • Experience working with older population, including frail and cognitively impaired adults preferred
  • Proficiency with Microsoft Office applications and other data-based applications (AlayaCare) is an asset
  • Valid driver's license and access to a reliable vehicle is an asset

Responsibilities

  • Provide information and referrals to clients to address their social, health care, personal needs and concerns as well as benefits application etc. whenever appropriate
  • Support clients/ families in navigating and coordinating services that meet their social, health, and personal needs—such as financial aid, legal support, housing, and medical treatment—enabling them to live fulfilling lives through integrated health and social care.
  • Plan, develop and lead community workshops and educational activities to promote healthy equity and raise awareness of available community resources and elder abuse prevention
  • Provide individual/group sessions to facilitate understanding & management of health issues, promote behavioural change, independence and autonomy
  • Respond to inquiries in person, by phone and via email
  • Perform other duties as required
