About The Position

Implement and manage a Community Schools initiative in Tumwater School District. Case manage students & families and leverage partnerships to remove barriers for academic success and youth wellbeing. Work in close collaboration with the Community Schools team, school leadership, teachers, counselors, and student support teams to minimize the gaps in education and health.

Requirements

  • Bachelor’s degree in Health Science, Community Health, Education, Social Work, Psychology, Human Services, or closely related field required. This requirement may be waived for individuals with 4+ years of youth or family case management, social service or education experience.
  • Minimum 2 years of experience working with high school aged students and/or young adults.
  • CPR/First Aid certification
  • Food Worker Card
  • Valid WA State Driver’s License and auto-liability insurance
  • Bilingual – Spanish/English fluency in speaking, reading and writing.
  • Foundational organizational, analytical, and problem-solving skills, with the ability to manage multiple priorities, meet deadlines, and perform effectively in a dynamic environment.
  • Foundational written and verbal communication skills, including the ability to communicate clearly, professionally, and effectively with staff, partners, stakeholders, and program participants.
  • Foundational understanding of workplace professionalism and non-discrimination practices, including appropriate handling of sensitive or personnel-related matters and adherence to equitable workplace standards.
  • Foundational professional conflict management skills, including the ability to navigate disagreements respectfully, maintain professionalism in high-stress situations, and escalate concerns appropriately when necessary.
  • High level of independence and self-direction, with strong follow-through, accountability, and the ability to contribute effectively within a collaborative team environment.
  • Basic proficiency in Microsoft Office and general computer applications, with the ability to quickly learn, adapt to, and effectively utilize new systems and technologies.
  • Demonstrated commitment to equity, inclusion, and systems change, with an understanding of systemic oppression and structural inequities and the ability to apply this awareness in professional practice.
  • Ability to represent TOGETHER! professionally, contributing positively to relationships with partners, stakeholders, community members, and program participants while upholding the organization’s mission and values.

Nice To Haves

  • Experience with case management models.
  • Experience in homelessness prevention and/or supporting populations experiencing unstable housing, particularly through the McKinney-Vento Act.
  • Knowledge, training, education and/or lived experience in trauma-informed care, adverse childhood experiences and youth mental health.

Responsibilities

  • Work with school staff, students, parents, community members and agencies to support students’ success in school. Develop and nurture relationships with these sectors.
  • Create school needs assessment and school plan in collaboration with school leadership and counselors. Design services around the assessment and plan.
  • Implement a menu of needs-driven, high-quality programs and services in adherence with the Community Schools Model, including but not limited to expanded learning opportunities, health services, parent/family engagement, adult education, direct material assistance and interventions/services targeted to low-income, homeless and/or chronically absent students.
  • Support both students and families with essential basic needs support, to include; housing instability, eviction prevention, transportation assistance, health insurance/government assistance navigation, and referrals to community organizations.
  • Leverage, develop, and/or strengthen and support programs that increase educational opportunities and social connections for families and students, both inside and outside the school system.
  • Case manage 40-60 students at Tumwater High School. Use the TOGETHER! in-take, follow up and other data tracking systems to track student progress and keep data organized for reporting requirements.
  • Assist Spanish-speaking students and families with school and systems navigation
  • Co-manage AmeriCorps service members, interns, and volunteers assigned to serve at this program site. Participate in volunteer-recruitment efforts for site’s program needs.
  • Manage a Community Resource Center in assigned program service building(s).
  • Generate support from in-kind donors to keep Community Resource Center stocked.
  • Serve as the point-person for agencies and programs interested in partnering with the school, facilitate alliances, and help to broker new partnerships with City agencies and non-profit partners that are aligned with the school’s goals and needs.
  • Participate in school student intervention teams.
  • Responsible for required community assessments, data collection, evaluation, program planning, strategic planning, adherence to the Community Schools model, and reporting.
  • Other duties as assigned, including performing own administrative work.
  • Attend agency training sessions and staff meetings.
  • Represent TOGETHER! and the Community Schools Program at events as needed.
  • Ensure implementation of policies adopted by TOGETHER!’s Board of Directors.
  • Follow the TOGETHER! Employed Personnel Policies.
  • Regular travel in personal vehicle between program/partner sites in Tumwater School District, TOGETHER!’s Headquarters office, and home office.
