About The Position

Under the direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC) provides direct patient care and participates in a therapeutic milieu within the scope of defined practice. This role involves supervising community activities, which may include transporting patients in hospital vehicles. The CRC must remain awake and alert at all times, assist patients with appointments, and maintain clear, concise, and timely documentation. Establishing helpful relationships with patients while maintaining professional boundaries is crucial. The position requires participation in patient rounds, status checks, and completing assigned patient care tasks. The CRC will work effectively to manage the therapeutic milieu, provide direction and supervision as needed, and follow established hospital and program policies, procedures, and protocols. Effective communication of patient milieu information and observations regarding medication side effects or patient changes to appropriate staff is essential. The role also involves encouraging patients in their activities of daily living, assessing sleep patterns, and completing mandatory annual in-service requirements such as CPR and de-escalation. Physical ability for participation in patient activities is required. The Behavioral Health Partial Hospital Program treats adults with mood and anxiety disorders, depression, personality disorders, bipolar spectrum disorders, and thought disorders, with an approximate participation length of three weeks. The program operates Monday through Friday from 8:30 am to 3 pm and emphasizes group therapy, structure, and skill-building using CBT, DBT, and ACT, integrating the systems model and positive psychology. Treatment includes medication consultations, individual therapy, case management, and aftercare planning.

Requirements

  • High School Diploma or Equivalent required
  • Driver's license (MA, CT, NH, NY, RI, VT) is required with the ability to satisfactorily meet and maintain hospital driving standards.

Nice To Haves

  • Bachelor's Degree Related Field of Study preferred
  • One-year patient care or related experience preferred, preferably in a psychiatric setting
  • 1-2 years preferred
  • Familiarity with Epic preferred.

Responsibilities

  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourages patients in their activities of daily living.
  • Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Physical ability for participation in patient activities.
