About The Position

Supportive Concepts for Families (SCFF) is seeking an experienced, passionate leader for the role of Community Program Coordinator. This full-time position, based in Reading, PA, offers DailyPay. The organization is dedicated to empowering individuals with intellectual and developmental disabilities to achieve greater independence, inclusion, and success. As part of the leadership team, the Community Program Coordinator will play a critical role in ensuring individuals receive person-centered services that help them achieve their goals and live fulfilling lives. Requirements

  • Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, or related field preferred.
  • Experience working with individuals with IDD, DD, and/or ASD.
  • Knowledge of Community Participation Support (CPS), In-home and Community Services, Individual Support Plans (ISPs), documentation standards, and billing requirements.
  • Must become ACRE Certified or CESP Certified within 6 months of hiring.
  • Ability to sit, stand, walk, bend, stoop, reach, lift, and climb stairs as needed.
  • Ability to drive a vehicle and travel frequently within assigned service areas.
  • Ability to operate standard office equipment and electronic documentation systems.
  • Ability to perform essential job functions with or without reasonable accommodation.

Nice To Haves

  • Minimum of three (3) years of supervisory or management experience.
  • Experience managing Employment Services, Community Participation Support (CPS), or In-Home and Community Supports (IHCS) programs.
  • Experience working with individuals with intellectual and developmental disabilities.
  • Strong organizational, communication, and leadership skills.
  • Ability to manage multiple priorities while maintaining compliance and quality standards.
  • Proficiency in electronic health records, documentation systems, and Microsoft applications.

Responsibilities

  • Oversight, coordination, and quality management of Community Participation Support (CPS) and In-Home and Community Support services for individuals with Intellectual Disabilities (IDD), Developmental Disabilities (DD), and Autism Spectrum Disorder (ASD).
  • Ensuring the delivery of person-centered services that promote independence, community inclusion, skill development, and achievement of individual goals.
  • Maintaining compliance with agency, state, and funding source requirements.
