About The Position

Lorenz Clinic is seeking a Community Partnerships Coordinator to manage and nurture relationships with referring organizations such as clinics, practices, schools, and programs. This role is crucial for maintaining trust between Lorenz Clinic and its referral sources by ensuring consistent communication, gathering feedback, and facilitating clinician outreach to these partners. The coordinator will manage a portfolio of relationships, arrange for clinician presentations, and track communication and feedback in a CRM system. This is not a sales role and is focused on relationship stewardship rather than quotas or commissions. The position requires daily travel within the southwest metro area.

Requirements

  • Two or more years of experience building and keeping professional relationships over time, and keeping records of them. Examples include development, stewardship, alumni or volunteer relations, community relations, provider relations, and referral or care coordination. Equivalent experience will be considered.
  • Daily use of a CRM, donor database, or similar contact-tracking system, and comfort with email and spreadsheets.
  • Clear, warm, professional communication in person, by phone, and in writing.
  • A record of follow-through: you keep a contact schedule without reminders, including when nothing comes back for weeks.
  • Ability to travel daily across your area, with visits to referring offices most afternoons.
  • A valid driver's license, a reliable vehicle, and proof of auto insurance.
  • Discretion with confidential information.
  • Successful completion of a background check.
  • Residence in or near your area of the southwest metro.

Nice To Haves

  • Experience holding a portfolio or a stewardship calendar in development, alumni relations, or volunteer coordination.
  • Moves management experience.
  • Experience in healthcare, schools, or social services, and familiarity with HIPAA.
  • Knowledge of the southwest metro's clinics, schools, and hospitals.
  • An interest in how families, schools, and medical offices work together, and in why that matters for a child's care.
  • A bachelor's degree, or equivalent experience.

Responsibilities

  • Keep a portfolio of referring offices in your area on a contact schedule: calls most mornings, visits to three or four offices most afternoons.
  • Ask, at every visit, call, and voicemail, how we are doing and what we could do better.
  • Record the answer the same day, and bring the themes to a weekly review.
  • Keep each relationship's record current in our CRM: who places referrals, how they like to hear from us, what they asked for last time, and what we promised.
  • Stay useful between asks: bring each office something it can use, such as this week's openings, an answer to last time's question, or a resource for the families they are worried about.
  • Send the Monday availability bulletin to referring offices and care navigators.
  • Confirm each morning that every family referred to us the day before was scheduled on the first call, and raise it the same day if not.
  • Send and record the clinical summary our clinicians write for every referred client, with the family's permission: at intake, at any change in level of care, and at discharge.
  • Set up conversations between our clinicians and referring physicians, school staff, hospital teams, and therapists, and make sure they happen.
  • Bring the weekly count to a weekly review, and help produce the monthly report that ties new clients to the relationships that sent them.
